All of you should by now have received your membership renewal packs for 2014 (the ASA membership year runs from January through to December each year).<\/p>\n

Please complete (at a minimum) the following documents:<\/p>\n

Membership form (1 per swimmer)<\/p>\n

Swimmer code of conduct (1 per swimmer)<\/p>\n

Parent code of conduct<\/p>\n

Medical form (1 per swimmer)<\/p>\n

Membership money \u2013 asked for annually \u2013 either \u00a320.50 (non competitive) or \u00a347.50 (competive) either cash (the correct money in an envelope would be wonderful for when there is no desk), cheque (made payable to PBSSS) or directly into the account \u2013 if paying in directly please make sure the name given is MEMB SwimmerName \u2013 where SwimmerName is the surname of your swimmer! Please indicate payment method on your membership form if possible. Remember you can split your membership money across three months (Jan, Feb, Mar) if that helps you.<\/p>\n

If anything has changed (e.g.moving from widths into lengths) you will also need to fill in a Standing Order Form \u2013 there should be one in your pack.<\/p>\n

If you wish to enter the club\u2019s Bonus Ball \u2013 \u00a34 a month, half the money goes out in prize money, the other half towards the running of the club \u2013 please complete the Bonus Ball Standing Order form.<\/p>\n

Wish to volunteer your time for the club \u2013 please complete the volunteer registration form (or for further info email\u00a0volunteer@perrybeechesswimming.co.uk<\/em>)<\/p>\n

Please keep the letter that you have received as the calendar for 2014 is on the reverse!<\/p>\n
